The Bill Chisolm Professional Service Award has been presented annually since 1994 in an attempt to recognize any individual who has significantly advanced the provision of athletic health care services to ethnically diverse individuals, or who has made a significant contribution to the professional development and advancement of ethnically diverse Athletic Trainers. The recipient is selected by the committee and is not required to be a member of an underrepresented ethnically diverse population.

Completing the Form: All nomination forms must be typed and complete in their entirety. Incomplete information or forms that are not signed by the sponsor or candidate could delay the nomination process significantly. Any category answered with "See Attached Resume" will be treated as a non-answer.
Familiarize yourself with what is being requested. Candidates are not expected to have experience in all areas and should not feel intimidated by having to leave some parts of the form blank.
Letters of recommendation: The candidate's application should include two letters of recommendation.
Sponsors are responsible for ensuring this occurs. Some nominees feel uncomfortable requesting such letters from colleagues. Others prefer to handle letter procurement themselves. With that in mind, we ask that the sponsor obtain these letters from the individuals the nominee designates, unless the nominee indicates s/he will do so.
Recommendation letters should conform to the following guidelines:
Letters should be printed on letterhead stationery and signed by the author. (We will not accept the letters electronically at this time.) Retired athletic trainers are not required to use business letterhead.
Two letters, one from the sponsor and one from any individual chosen by the candidate.
Résumé or Vita: A current résumé or vita must be returned with your nomination form. This is to supplement the information provided in the nomination form.
- Deadlines: All materials requested, including the nomination form, supporting documentation and letters of recommendation are due in hard copy to the NATA Ethnic Diversity Advisory Committee by January 2, 2010 (Sorry, but we are unable to accept electronic submissions.) You are welcome to contact the chair Keith Garnett to confirm receipt of your complete package.
- The Evaluation Process: The Ethnic Diversity Advisory Committee Honors & Awards Sub-Committee will review completed nomination forms.
Notification : Recipient will be notified of the committee's selection in April.
